Making collaborative care a reality for people with long term conditions – 30 June, London

The Coalition for Collaborative Care are hosting an event on Monday 30 June from 10am to 4.30pm at Mary Ward House, London to share progress, discuss how they can continue to build the coalition and put aspirations into practice.  

The coalition is a growing group of individuals and organisations across the health, social care and voluntary sectors who want to work collectively to make person-centred, coordinated care a reality for people living with long-term conditions.

They are aiming to involve around 80 people from across health, social care and voluntary sectors, a mix of professional disciplines, patients and carers.

To ensure that the valuable views and experiences of patients and carers are included at the event they are looking for around 10 patients/carers with lived experience of managing a long-term condition and are comfortable talking about their experiences at the event.
