It is time to think differently about health and social care. Although good process has been made in improving NHS performance in the past decade, the health and social care system faces unprecedented funding pressures and significant future challenges. Fundamental change is needed to respond to the needs of an aging population, the changing burden of disease and rising patient expectation.

Integrated care must be delivered at scale and pace. This requires work across large populations at a city and county-wide level. No single ‘best practice’ model of integrated care exists and processes are needed to support rather than inhibit organisations working together

This briefing will look at what Integration and Transformation means and what the role of the VCS might be.

Background

“Integrated Care” has become the buzzword in health and social care circles, successfully fuelled by the Department of Health’s invitation to local areas to become health and social care integration pioneers, and the subsequent announcement of the £3.8billion Integration Transformation Fund to support the integration of health and social care services locally.

In order to access this money local authorities and the local NHS will have to commit to joint commissioning, better data-sharing, seven-day working across health and social care services and the protection of social care services, and will require an accountable lead professional for packages of integrated care.
